| EJECTION REPORTING UTILITY - BASEBALL |
| Please report ejections to the District using this utility. The Umpire is our preferred method of reporting, however, a District Staff member or Local League staff member may also report the ejection. The Player or Coach who was ejected may not use this form to report the ejection. NOTICE: Ejections must be reported to the local league President(s) as per Little League Rules & Regulations. This reporting utility reports to the District and does NOT report to the local league President(s). You will be sent a copy of the report you file below via email to the email address you provide (if valid). PLEASE FORWARD A COPY OF THE EJECTION REPORT TO THE LOCAL LEAGUE PRESIDENT(S). You may also use this form to report local league ejections so you have a convenient record to forward to your local league President, however, local league games are a matter for your local Board of Directors, not the District. |

| INSTRUCTIONS |
| Please follow these instructions when filling out the Baseball Ejection Report. |

| TYPE OF GAME BEING REPORTED |
| Indicate the type of game that was being played for the ejection you are reporting. Interleague Play Game - TOC Game - All-Star Game - League Game (Regular Season) |

| DIVISION OF PLAY |
| Indicate the division / level of play for the ejection you are reporting. |
|
| REPORT TYPE |
| The first time you report an ejection, it is a "Ejection Report". To correct a previously filed Ejection Report, file a "Ejection Correction". |

| DATE OF EJECTION |
| Indicate the Month / Day & Weekday the ejection occurred on. |
|
| GAME LOCATION |
| Select the League where the game was played. If the game was not played at an FLD6 League, select "Other Location" from the drop-down menu and then type the location the game was played at in the "Other Loc." field. |

| HOME TEAM INFO |
| List the Home Team's League, team name, and Manger's name. If the Home Team is not from a FLD6 league, type the District & League Name in "Other League" field. List the names and jersey numbers of the players ejected (if any). List the names of the coach(es) ejected (if any). List details of ejection(s) in the "Comments" below. |

| VISITORS TEAM INFO |
| List the Visitor's League, team name, Manager's name. If the Visitors are not from a FLD6 league, type the District & League Name in "Other League" field. List the names and jersey numbers of the players ejected (if any). List the names of the coach(es) ejected (if any). List details of ejection(s) in the "Comments" below. |

| UMPIRES |
| Please list the names of the Umpire(s) for the game in which the ejections occurred. |

| REPORTING PERSON CONTACT INFO |
| Please give us your contact information in case we have any questions related to this game. The Home Team Manager is who should be reporting games to the District Interleague Play Game Database. |

| EJECTION COMMENTS |
| Let us know if you would like to record any additional comments regarding this game. These comments are for the database only. If you feel you have an issue that needs to be addressed by the District Staff, please email an "Issue" to the Staff. Information and the email address can be found in the Interleague Play Rules (PDF). |

| SUBMIT DATA |
| Click-on "Submit Data " to record your ejection data to the District Interleague Play Ejection Database. If you would like to clear this form of all data you have entered and NOT record the data, click-on "Clear Form". If you have submitted an ejection to the database and discover that there were errors in the data, file a new Ejection Report for the ejection and list the "Report Type" as a "Ejection Correction" (see Report Type above). |
