| Please report your Distrcit Intereleague Play games using this utility. This record will help the District retain and verify records of Interleague Play games. These records will help us verify Little League games-played requirements to qualify teams & players for All-Stars, as well as facilitate checking "Official Game Books" for post season play. YOU WILL NOT RECEIVE CREDIT FROM THE DISTRICT FOR GAMES PLAYED THAT ARE NOT LISTED IN THIS DATABASE |

| If there were any ejections related to this game, an "Ejection Report" must be filed with the District. |
| INSTRUCTIONS |
| Please follow these instructions when filling out the Baseball Game Report. |
| SOFTBALL DIVISION |
| Indicate the division of play for the game you are reporting. |
| REPORT TYPE |
| The first time you report a game, it is a "Game Report". To correct a previously filed Game Report, file a "Game Correction". |
| DATE OF GAME |
| Indicate the Month / Day & Weekday the game was played on. If the game was incomplete or postponed for any reason, use the date the game was started on (date the first pitch was thrown). |
| GAME LOCATION |
| Select the League where the game was played. If the game was not played at an FLD6 League, select "Other Location" from the drop-down menu and then type the location the game was played at in the "Other Loc." field. |
| COMPLETE GAME ? |
| If the game was a "Regulation Game" completed per LLB rules, list as a "Complete Game". If the game was not completed to a Regulation Game for any reason, mark game as "Incomplete" (weather, curfew, delay, not enough players, etc.). |
| WHO WON THE GAME ? |
| Tell us who won this game if the game was completed or forfeited by either team. List the game as a tie or incomplete game as appropriate. |
| HOME TEAM INFO |
| List the Home Team's League, team name, score, and Manger's name. If the Home Team won by Forfeit, list their Score as "7". If the Home Team lost by Forfeit, list their Score as "0". If the Home Team is not from a FLD6 league, type the District & League Name in "Other League" field. |
| VISITORS TEAM INFO |
| List the Visitor's League, team name, score and Manager's name. If the Visitors won by Forfeit, list their Score as "7". If the Visitors lost by Forfeit, list their Score as "0". If the Visitors are not from a FLD6 league, type the District & League Name in "Other League" field. |
| ANY EJECTIONS ? |
| Let us know if there were any Player(s) or Coach(es) ejected related to this game. If there were any ejections, an "Ejection Report" must be filed with the District. |
| REPORTING MANAGER CONTACT INFO |
| Please give us your contact information in case we have any questions releated to this game. The Home Team Manager is who should be reporting games to the District Interleague Play Game Database. |

| SUBMIT GAME DATA |
| Click-on "Submit Game" to record your game data to the District Interleague Play Game Database. If you would like to clear this form of all data you have entered and NOT record the game data, click-on "Clear Form". If you have submitted a game to the database and discover that there were errors in the data, file a new Game Report for the game and list the "Report Type" as a "Game Correction" (see Report Type above). |
